Triple M (Me, My Family, My Community)
Outreach Based Casework & Case Management Program
Our Triple M Program caters for young people aged 12-25 within the Central Coast who are vulnerable or at-risk due to mental health issues, disabilities, homelessness, substance abuse, barriers to employment and limited community support. The program engages youth through Mentoring, Casework and Case Management approaches through a strength based model. These issues will be addressed through the perspective of "Me, My family, My Community" (Triple M) .
The Mentor/Caseworker works alongside the participants utilising strength based approaches to empower, build resilience and ownership of each goal created. The mentor meets with the participant on a monthly to fortnightly basis to work through and support the participant with engaging in a variety of services which could include: employment services, resume writing and employment preparation, educational support services, Drug and Alcohol services, counselling, accommodation support (referrals to refuges/Housing Application process/Centrelink) and court support.
